I provide post-hospitalization and rehabilitation support focused on recovery while encouraging movement, mobility, and daily activities at a pace that feels safe, comfortable, and appropriate for each individual. This service may include: • Assistance with transfers and safe repositioning • Support with ambulation and mobility within the home • Exercise and movement reminders as directed • Positioning support to promote comfort and circulation • Carrying out care plans and written instructions from licensed providers • Reinforcement of physical and occupational therapy goals • Observation and reporting of changes in mobility or tolerance • Encouragement and motivation throughout the recovery process Rehabilitation support is delivered with patience, respect, and professionalism, creating a supportive and encouraging environment where clients feel safe, motivated, and empowered as they work toward recovery goals.
